Do you take insurance?

You should be at the front of your care. I believe in being direct, honest, and transparent. As an โ€œ๐™ค๐™ช๐™ฉ ๐™ค๐™› ๐™ฃ๐™š๐™ฉ๐™ฌ๐™ค๐™ง๐™ โ€ provider, I do not seek reimbursement from your insurance provider for my services. Payment is due at the time of service.

It is the goal of ๐™•๐™๐™ช ๐™‹๐™๐™ฎ๐™จ๐™ž๐™ค ๐™Ž๐™ค๐™ก๐™ช๐™ฉ๐™ž๐™ค๐™ฃ๐™จ ๐™‡๐™‡๐˜พ to provide elite level care which is highly personalized and well communicated. I am here to listen to you and help guide you to the best of your ability without taking you out of your activities. This model bypasses unnecessary medical appointments and accelerates your return to sport, pain free activity, and improved function. Our main focus becomes your health and goals, not insurance paperwork. If we are not the right PT for you, we will use our network to get you in front of the ๐™ง๐™ž๐™œ๐™๐™ฉ person, and not just the next one.

The current system of insurance creates barriers to care including high patient volume, decreased overall physical therapy experience, increased appointments within the medical system, decreased ability for 1 on 1 care, inability to treat multiple body regions at one time, and low reimbursement to the provider. These pressures on providers may result in double booking appointments, decreased time for communication with other practitioners in your care team, and the administration of non-skilled treatment when it is not necessary just for the sake of profit. Our promise to you is that every session you have with us will be productive and you will leave feeling accomplished. We are not your standard PT clinic that watches you do the same exercises over and over again.

When requested, we will provide a โ€œ๐™จ๐™ช๐™ฅ๐™š๐™ง๐™—๐™ž๐™ก๐™กโ€ for you to submit to your insurance agency for potential โ€œ๐™ค๐™ช๐™ฉ ๐™ค๐™› ๐™ฃ๐™š๐™ฉ๐™ฌ๐™ค๐™ง๐™  ๐™ง๐™š๐™ž๐™ข๐™—๐™ช๐™ง๐™จ๐™š๐™ข๐™š๐™ฃ๐™ฉโ€. It is recommended you call your insurance agency to ask about out of network benefits. Is it possible to get my insurance to pay for my visits?

Your insurance card will have a member line phone number printed on it, you will need that alongside your ID number.  Ask to speak to a customer service member and be sure to go over all the following:

  • Do I receive Out of Network benefits for Physical Therapy?

    • If yes, what is my percentage of coverage? (Most PPO insurances will reimburse up to 70% of the โ€œeligible costโ€). You can ask specifically โ€œIf my PT charges $150/hr, how much can I expect to be reimbursed?โ€

  • Do I have a deductible?

    • If so, how much is it?

    • Have I reached my deductible?

  • Is there an annual dollar amount limit for coverage? Some insurances will only pay up to a certain amount per calendar year for PT services. Be sure to what that dollar amount is.

  • Do I need a referral from a doctor to be reimbursed?

    • If yes, does that doctor need to be my primary physician?

  • Do I need prior authorization (or pre-authorization) filed before starting Physical Therapy?

    • If yes, how can I get this?

  • Is there a specialized form that needs to be completed for reimbursement or can I simply submit the โ€œsuperbillโ€ provided by my PT?

    • If special form is needed, where is that form located?

    • What is the mailing address where the form needs to be sent?

    • Am I able to file online?

Do I need a physicians referral to see you?

Hawaii is one of 20 states that allow patients total, unrestricted access to a physical therapist. No type of physician referral is required for a physical therapist to treat a patient whether it is for an injury or just for preventative/recovery work.

While direct access to a physical therapist is not restricted in Hawaii, sometimes a physicianโ€™s referral is needed in order to get reimbursed for our services. Please see the above FAQ regarding insurance reimbursement if you have a โ€œPPOโ€ plan with โ€œout of networkโ€ benefits.
